No study has ever shown that creatine causes hair loss. The entire rumor traces back to one 2009 paper on 20 rugby players that measured a hormone, not hair. Nothing since has replicated even that hormone finding, and no research has ever linked creatine supplementation to thinning, shedding, or a receding hairline.

I get this question more than almost any other supplement question, and it usually arrives the same way. A guy in his late thirties tells me he wants to start lifting seriously again, I mention creatine, and he goes quiet for a second. Then: "Doesn't that stuff make your hair fall out?"

He read it on Reddit. Or a guy at his gym told him. Or a YouTube video with 2 million views mentioned it in passing as a known side effect. In 13 years of coaching at CoachCMFit, I have watched this one rumor talk more people out of the single most well-researched supplement in sports nutrition than any real side effect ever has.

Where Did the Creatine Hair Loss Rumor Start?

In 2009, researchers at Stellenbosch University in South Africa published a small study in the Clinical Journal of Sport Medicine. Twenty college-aged rugby players took 25 grams of creatine monohydrate per day for seven days, then dropped to 5 grams per day for another 14 days. The researchers measured testosterone and dihydrotestosterone, better known as DHT.

DHT is the hormone most closely associated with male pattern baldness. In people who carry the genes for it, DHT binds to receptors in the hair follicle and slowly shrinks it over years until it stops producing a visible hair. That process is called miniaturization, and it is the actual mechanism behind the receding hairline you see in men and the diffuse thinning you see in women.

So when this study reported that DHT climbed 56 percent during the loading week and stayed 40 percent above baseline during maintenance, the internet did the math on its own. Higher DHT means faster balding, creatine raises DHT, so creatine makes you bald. Clean logic. Wrong conclusion.

The Research

The 2009 van der Merwe study from Stellenbosch University measured hormones in 20 rugby players over three weeks. It did not measure hair count, hair density, follicle miniaturization, or shedding rate at any point. Not once. The word "hair" appears nowhere in its results, because hair was never an outcome the study was designed to look at.

The finding has also never been replicated. Roughly a dozen later studies measured testosterone and DHT in creatine users and found no meaningful change in either. A single unreplicated hormone result from a 20-person study is the thinnest evidence a claim can rest on, and it is the whole foundation of a rumor that has been circulating for 17 years.

Even the DHT number itself deserves context. The players started with DHT levels in the lower end of the normal range and finished still inside the normal range. A 56 percent jump sounds enormous until you learn it moved the average from below the middle of normal to the upper end of normal, which is roughly the difference between two random healthy men standing next to each other.

What Does the Newer Research Say About Creatine and Hair?

The evidence has only gotten cleaner since. Systematic reviews of creatine safety have looked specifically for hair and hormone effects and come up empty, and a 2025 randomized controlled trial went a step further by measuring hair follicle health directly rather than inferring it from a hormone panel. It found no evidence that creatine supplementation caused or accelerated hair loss.

That is the part people miss. We are no longer arguing from the absence of evidence. Researchers went looking for this effect on purpose, with the right measurement tools, and it was not there.

Name the real villain: genetics. Androgenetic alopecia affects roughly half of men by age 50 and a significant share of women after menopause. It is driven by inherited follicle sensitivity to DHT, not by how much DHT you produce. Two men with identical hormone levels can end up with completely different hairlines because one inherited sensitive follicles and one did not. Creatine does not change what you inherited.

Why Do People Think They Lost Hair From Creatine?

Because the timing lines up perfectly, and that is a trap.

Think about when someone starts creatine. It is almost never a random Tuesday. It is the same week they start a serious training program, cut calories, add cardio, chase a new job, sleep less, and generally put their body under more stress than it has been under in years. Any one of those things can trigger telogen effluvium, a temporary shedding phase that shows up two to four months after the stressor.

So four months into a hard training block, a guy notices more hair in the shower drain and remembers he started creatine. The correlation feels obvious. The actual causes are more likely sitting somewhere in this list:

Notice that four of those five are things a coach can actually help you fix. That is why my first question when a client raises this is never about the creatine tub. It is about sleep, calories, and the last time they got bloodwork.

Should You Stop Taking Creatine?

If you are not experiencing hair loss, there is nothing here to act on. Keep taking it.

If you are experiencing hair loss, stopping creatine will almost certainly not fix it, because pattern hair loss is progressive and driven by genetics. See a dermatologist and get thyroid, iron, and ferritin checked. If you want personal certainty after that, run your own test: stop creatine for three to four months, since that is the lag time on any hair cycle change, and see whether anything shifts. Most people find nothing does, and go back to it.

What you give up by quitting is real, and worth naming. Creatine is the most validated performance supplement in existence, and its benefits reach well past the weight room. It improves high-intensity work capacity, supports strength gains, and shows up in research on mood and cognition. How Do You Know If Creatine Is Actually Working?

Not by feel. Creatine produces no buzz, no pump you can reliably notice, and no obvious sensation of any kind. It shows up in your training log.

Take the weight you can currently press for 8 reps. Four weeks into consistent creatine use, with training and nutrition otherwise unchanged, you should be getting 9 or 10 reps at that same weight. That is the effect: a slightly higher ceiling on repeated hard efforts, which compounds into more total volume, which compounds into more muscle over months. Small edge, applied relentlessly.

  5. If your hair is thinning, book a dermatologist and get thyroid, iron, and ferritin tested. That is where the answer usually is.

The fitness industry runs on fear because fear sells more than nuance does. A supplement that costs 12 dollars a month and works has no marketing budget defending it, so a 17-year-old rumor built on one unreplicated hormone measurement gets to outrank two decades of safety data. Do not let that decide your training for you.
